I know you've probably heard this before and what not but I went to boarding school and my roommate had anorexia and missed out on half of our junior year because she was in the hospital. Her body weight got to a point that was so low that she went into cardiac arrest and almost died. I know a lot about anorexia, from her, and I know that it is an illness and not a choice. However, I want you to know that it is an illness that can be treated and you should see a doctor about it. My friend almost died at 16 years of age. To answer your question, it's best to lose weight over a short period of time. Healthy weight loss consists of 1-2 lbs per week. However, you should see a doctor about this given your condition. Waiting until it's an emergency is not a good idea. My friend was fighting for her life, waiting for an emergency and she lived- but everyone does not. You need to go to a clinic or something. They have many eating disorder clinics and my former roommate is now back to her original weight and not ashamed. They are very helpful.
You need protein, and lots of it. Don't forget to load up on fibre, lots of lifters forget this. Good carbs are a must.
Based on the reading I have done, strength training is the way to go.
It sounds fine! How much longer is it supposed to be?, or do you just need to supply a statement? If that is all it is, then you are done. Your statement covers it all.
But maybe they are interested in how you will change. Are you going to stop having a snack after school, and go outside and ride your bike or play basketball? What are you going to do to actually MAKE the change. You could expand it quite a bit if you have to add that. It all depends on what they are actually asking you for.

I am so sorry to hear about your situation and I definitely can understand your frustration. With Psychiatric medicine, it takes about 4 – 6 weeks before you began to feel normal, and the medication is a hit and miss. You might have to try 2 or 3 different ones until the Dr. and you have the right combination of medications. The affect of feeling like being in a dream state is normal when taking certain medications. I have gone through the same thing, and even now, if I forget to take my medicine for over 2 days I start to feel funny again. Your situation with the weights may just have happened during the same time of your break down. That is what this sounds like. A mental breakdown, which can be healed in time. Stress, Anxiety and Depression bring on all the symptoms you mentioned. Even the numbness in your arm/hand. The new glasses is another thing your going to have to adjust your eyes too, so I would suggest that you don't wear the glasses just yet, but work on getting the right combination of medicines to help you feel normal. Too much at one time might make things worse. You are not crazy. You might not have been aware of how much stress you were under during the time you were lifting and in your mind you have put all this together as one problem. It was just a coincidence that all this was going on at the same time. Your Dr. can give you something to help u sleep and also to get rid of the head aches. This is just your cross in life the bare and you must stay strong and get thru this, so you can go on with your life. Like me, you might end up needing medication for the rest of your life, but so what. Please don't give up. You are so close to a break through, eventually the haze will clear up and you will begin to feel better. Don't just go by what you feel. Go by what you Know. Your mind is everywhere right now because of what is happening, but ignore the thoughts that you know are not right! Hang onto reality and do what is right no matter what you feel. You will get through this, and anytime you need to vent about it. write. May you receive the Grace to go through until you can get healed.
